Glad to hear your SK cancellation went thru. I sent a certified copy of my father's death certificate along with a letter stating the 5 W's (who, what, when, where and why) of my request to the FCC. It was recv'd by them Jan 26th and the request was recorded on Feb 11th. Not sure of any other formats that might have been useable. I figured that dealing with the USG that the basic real deal document would get through the fastest. Took 16 days from receipt till processed if you can count that as fast???

In my vanity application where I applied as a 'Close relative of the former holder' I did not submit any supporting documentation nor was any requested. I have it if needed, of course - my birth certificate with Dad's name on it.
